Efforts to complete the 2019 Communuity Health Needs Assessment (CHNA) is well underway. Listening sessions (previously known as focus groups) have begun and we are capturing feedback from our community on the health disparities in this region. Listening sessions are currently booked through April.

We are very excited to announce that NEW this year we have launched a Community Survey to inform the Community Health Needs Assessment. We encourage everyone who lives within the Greater Lowell area to complete the survey. We want your voice to be heard! The data collected through this survey will help identify the top health needs for our community as well as help with the priority areas of the 2020 Community Health Improvement Plan (CHIP). 
 
The GLHA is encouraging community agencies & organizations to share this survey with their staff and clients. There are many ways in which individuals can complete this survey. 
 
There are several ways in which you can contribute to this effort: 
 
1. Distribute the digital survey link to your community networks. 
The online version of the Community Health Needs Assessment is accessible via secure link https://bit.ly/2Vo87HQ
 
This survey can be completed via mobile phone or on a computer. Currently, this survey is only available in English. (Spanish will be online soon)
 
2. Volunteer your organizational site as a hard-copy location. Hard copy, paper versions of the survey are available in English, Khmer, Spanish, Portuguese, Swahili and Arabic. If you volunteer your site as a hard-copy location, a member of our team will provide paper copies of the survey in the languages of your choice, as well as a secure drop off location where completed surveys can be left for weekly pick up. We will also provide a short training to your staff regarding appropriate survey protocols to ensure secure, confidential collection of data. 
 
3. Host an oral interview for survey participants. If your organization has regular group meetings with community members, especially community members who would benefit from an oral translation of the survey, volunteer to host an oral survey meeting. During an oral survey, a member of your team will read the survey questions out loud to participants in the most appropriate language for your settings, and participants will record their answers in either paper copies or an electronic version of the survey. This is most important for our community members who are minority language speakers, or who may not be fluent in written language. 
 
4. Offer the survey at your planned events. If your organization will be hosting or attending upcoming community events between now and April 2019, please request hard copies of the survey, or bring a copy of the survey QR code (attached) so that it can be scanned and completed via mobile phone. A member of our team will coordinate with you to pick up completed hard copies of the survey as needed. 
 
A few notes about this survey: 
•    Survey participants should be 18 years old or older, and live in the Greater Lowell area. 
•    Participants can opt in to be entered to win one of five $50 gift certificates as a thank you for participation. 
•    Participation in this survey is confidential and anonymous; no answers will be connected back to individuals and the survey does not require names or addresses.
