Do I still have to visit the police station to file a Motor Vehicle Crash Operator Report?
No, you do not have to visit the station to file the report. Individuals can obtain the Motor Vehicle Crash Operator Report online. Please visit www.mass.gov/doc/motor-vehicle-crash-operator-report to obtain a copy of
the report. Please note that this report must be completed within 5 days after the crash (unless the individual
was incapacitated).

I completed the Motor Vehicle Crash Operator Report. Where do I send it?
After completing the Commonwealth of Massachusetts Motor Vehicle Crash Operator Report,
please mail or deliver one (1) copy to the local police department or state police in the city or
town where the crash occurred, mail one (1) copy to your Insurance Company, and mail one (1)
copy to the RMV at the following address: Registry of Motor Vehicles Crash Records P.O. Box
55889 Boston, MA 02205-5889.

Reports can be picked up Monday - Friday between 8 a.m. to 4 p.m. To assist the Records Staff, please know the date and the location of the incident, the officer's name and the names of the persons involved.
